The 5 questions* below are answered Yes or No. There is no need to elaborate on reasons for your answers.

Yes is scored 0. No is scored 1. Higher score with better health.

If person reports NO problems interfering with quality of life in the 5 questioned areas:

  1. Physical
  2. Psychological/Emotional
  3. Social/Making Friends
  4. Work/School/Play
  5. Outlook on Life

they will score a 5. If you answer YES to any questions your score will be between 0-4. These results may show some underlying issues in your daily life that could possibly be causing health issues. This tool will be useful in helping diagnose and treat health issues, and ultimately lead to better overall healthcare. Discussing your answers with your healthcare provider will not only help earlier diagnoses but reduce health costs.
